Appointments can be made Monday through Thursday, 8:00 am – 4:00 pm and Friday 8:00 am – 2:00 pm.
When scheduling an appointment, the receptionist will gather demographic information, insurance information, and your medical records to ensure a productive first visit.
New patients should arrive 30 minutes prior to their scheduled appointment time to ensure adequate time to complete forms that become part of their electronic medical record. Established patients should arrive 15 minutes prior to their scheduled appointment time. Patients who arrive more than 15 minutes after their scheduled time, will either be re-scheduled or if available, will be evaluated during an open time slot that day. Please understand that there may be a lengthy wait if you opt to be seen under this circumstance. If you are unable to keep an appointment, please contact us at least 24 hours in advance so we may use this time for other patients.
If you are scheduled for a procedure and you need to cancel, please give us 48 hours notice.
